Veteran Care Navigator – Full time in Paw Paw office

As part of the Certified Community Behavioral Health Clinic (CCBHC), the Veteran Care Navigator will help customers navigate and access community services, other resources, adopt healthy behaviors, improve health outcomes, and reduce healthcare costs. The Navigator will identify and link veterans and military families to needed resources and services in the area, make appropriate referrals, coordinate services with customers, and provide follow-up. Frequent travel to various locations throughout the county required to provides services. Duties include: offering emotional support, encouragement, and practical assistance to individuals and families of veterans and those who have served the country; utilizing lived experience to engage and support recovery in others; helping customers meet treatment and life goals while reducing barriers to those goals; and working closely with resources such as the Veterans Administration & Buddy to Buddy to engage in extensive outreach to promote mental health treatment to veterans and military families. Requires: High School Diploma or GED with at least 1 year of military service lived experience and willingness to share personal story with others.  Must have or be able to complete training to obtain Michigan Peer Support Specialist certification.

RN Care Coordinator – Full time in Paw Paw office

As part of the Certified Community Behavioral Health Clinic (CCBHC), the RN Care Coordinator will help customers navigate and access community services, other resources, adopt healthy behaviors, improve health outcomes, and reduce healthcare costs. The Coordinator will identify and link customers with medical resources in the area, make appropriate referrals, coordinate services, and provide follow up with customers. Ensures customers receive appropriate and necessary monitoring for Medication Assisted Treatment (MAT) clinic. May travel to various agency sites through the county to provide services. Will provide CPR/First Aid training to staff. Requires Michigan RN license; knowledge of nursing practice as it relates to working with individuals with mental illness & substance use disorders; and ability to obtain CPR/First Aid Instructor certification within 6 months of hire.

Substance Use Disorder Recovery Coach – Part-Time in Paw Paw
Part-time at 30 hours/week. Peer position will work closely with customers and SUD services staff to assist individuals in achieving their stated recovery goals, manage their emotional and behavioral symptoms, and supports development of independent living skills, based on treatment plan. Provides services to promote recovery through teaching and role modeling with customers. Services may be provided in the community or at agency work sites. Requires certification as Certified Peer Recovery Mentor or have ability to attain; minimum of a high school diploma or GED; valid Michigan driver’s license; and SUD lived experience, with two years sobriety.

Mobile Crisis Peer Support Specialist – Full time in Paw Paw
This Peer worker will provide support and hope through direct assistance to customers and their natural supports during a mental health crisis. Provides encouragement and valuable information to connect customers and their families to community resources. Assists in overcoming barriers to treatment and recovery. Services may be provided within the community or at agency work sitesServices may be provided in the community or at agency work sites. Requires certification as Certified Peer Support Specialist or ability to complete training to obtain; minimum of a high school diploma or GED; valid Michigan driver’s license; and past or present customer of mental health services and be willing to share personal recovery story.

Behavior Specialist – Full time in Paw Paw
Master’s level clinician needed to provide assessments, testing, behavior analysis, counseling, and psychotherapeutic services to assigned adults and children with developmental disabilities and challenging behavior. Will write, in-service and monitor Positive Behavior Programs. Provides training to the community and/or inter/intra-agency entities. Serves as a resource to staff and the community on Positive Behavior Supports/Interventions and issues regarding developmental disabilities and mental illness. Requires Master’s degree in Psychology with LLP licensure with at least 1 year of experience working with persons with I/DD developing behavior treatment plants and positive support strategies. Experience providing assessment/testing services and/or developing effective behavior treatment plans preferred. 

Home-Based Clinician/Youth & Families – Full time in Hartford
Master’s level clinician needed to provide home and community-based intervention and support to families with infants, young children, and youth with mental health issues. Outreach services help to promote positive parenting attitudes and skills, improve family relationships, encourages use of positive approaches to child behavior/discipline, and interventions the caregivers may utilize to support healthy social and emotional development of the child, and to remediate problem behaviors. Provides Home-Based services including assessments, clinical intervention, identifying/intervening in mental health needs, alleviating crisis, and ensuring stabilization in the community for assigned youth and their families. Requires Master’s degree in a mental health related field with appropriate state licensure; at least 1 year of experience working with families with young children; and demonstrated knowledge/training of social work practice as it relates to working with families of young children, including the ability to understand age-appropriate functioning, developmental stages, and the impact of the parent/child relationship, in order to provide assessment, diagnosis, appropriate intervention, and referral.

 School-Based Clinician – Full-Time in Bangor
Master’s level clinician needed to work in the Bangor Schools to provide clinical therapy and/or case management services to students; identify/intervene in mental health needs, alleviate crisis and ensure stabilization of youth/families in the community, school and/or agency sites. Will provide assessment, develop service/treatment plans, and review effectiveness of goals/services. Must have Master’s degree in social work, psychology or counseling with State licensure. At least two years’ experience in mental health assessment and treatment of youth and their families with demonstrated diagnostic and case reporting skills.
