Behavioral Health & Wellness for Adults
Everyone deserves support. We offer inclusive, whole-person help for all individuals in our community—no matter where you are on your journey. From prevention and assessment to crisis response, treatment, and recovery, our team walks with you every step of the way—building hope, healing, and a stronger tomorrow.
Our Adult Case Management team helps adults living with serious mental health conditions connect to the care, resources, and support they need. We work alongside you to create a plan that supports your goals—whether that’s finding the right medical care, accessing community programs, building job skills, or managing daily needs.

*Case Management also serves individuals who have additional challenges, such as substance use concerns, both mental illness and developmental disabilities, or medical conditions that make them more vulnerable. When special needs are identified, our staff works closely with other professionals to make sure your service plan is well-coordinated.
Our Van Buren Community Mental Health ACT teams provide intensive, community-based support for adults living with serious and persistent mental illness. Designed for individuals who may need more than traditional outpatient care, Assertive Community Treatment (ACT) offers a full range of services—right where you live, work, and spend your time.
With help available 24/7, our teams work closely with you to manage symptoms, build daily living skills, connect with resources, and reach your personal goals. ACT is about meeting you where you are, offering consistent, compassionate care that helps you stay healthy, independent, and connected to your community.
Our Care Coordination services connect you to the right care at the right time. We work closely with your healthcare providers, community resources, and support network to make sure your mental health, physical health, and social needs are all addressed. Whether it’s scheduling appointments, sharing information between providers, or helping you navigate complex systems (like finding resources and benefits), our VBCMH team is here to keep your care organized, accessible, and centered around your recovery goals.
Community Support Services (CLS) at VBCMH provide hands-on help and skill-building so adults can live as independently as possible at home and in the community. Guided by your person-centered plan, trained staff can support daily routines (like meal prep, light housekeeping, personal care), getting to appointments or activities, practicing coping and social skills, and building confidence with tasks such as budgeting and time management.
Services are flexible—days, evenings, or weekends—and can be short-term or ongoing. We also coordinate closely with your VBCMH care team, so your supports are consistent, practical, and aligned with your goals.
Our Elder Outreach Services bring compassionate mental health support directly to older adults in their homes, care facilities, or community settings. We understand that aging can come with unique challenges—such as changes in health, mobility, or independence—and our goal is to help you navigate them with dignity and confidence. Whether it’s counseling, connecting to resources, or simply having someone to talk to, our team is here to ensure older adults feel supported, valued, and empowered to maintain their quality of life.
Our Hospital Care Management services help individuals during and after a hospital stay for a mental health crisis. We work closely with you, your loved ones, and hospital staff to ensure you have the support you need for a safe and smooth transition back into the community or your home.
From creating a follow-up care plan to connecting you with ongoing mental health and wellness services, our goal is to reduce the risk of future hospitalizations and help you continue your recovery in a supportive environment.
Our Integrated Employment Services help adults with mental health needs explore, prepare for, and succeed in meaningful work. We believe employment can be an important part of recovery, independence, and self-confidence. Our team offers personalized support—from identifying career goals and building job skills to connecting with employers and providing on-the-job coaching. Whether you’re entering the workforce for the first time or returning after time away, we’re here to help you find and keep a job that’s the right fit for you!
Benefits Coaching
VBCMH Benefits Coaching services help you understand and make the most of the benefits available to you, such as Social Security, Medicaid, or other assistance programs. We’ll guide you through the process, answer your questions, and work with you to ensure your benefits support your goals for health, independence, and financial stability.
Outpatient Therapy (OT) is available to adults of all ages who need support with behavioral health, wellness, or a substance use disorder and/or co-occurring (dual diagnosis) disorders. Our outpatient therapy services offer a safe and supportive space to work through life’s challenges, improve your well-being, and reach your personal goals.
Therapy can be provided one-on-one, with your family, or in a group setting—depending on what best supports your needs. Our skilled therapists use evidence-based approaches to help you manage stress, build coping skills, strengthen relationships, and navigate mental health concerns. Whether you’re seeking short-term guidance or ongoing support, we’re here to support you every step of the way.
Our VBCMH Psychiatric Care team provides compassionate, professional support for adults with ongoing mental health needs. Our psychiatrists and psychiatric nurse practitioners offer thorough evaluations, medication management, and regular follow-up to help you feel your best. We work closely with your broader health care team, from therapy to case management and other supports—so your treatment plan is connected, personalized, and easy to navigate.
Psychosocial Rehabilitation (PSR) at VBCMH helps you build the skills and confidence to live well in your community. Through goal-focused groups and one-to-one coaching, we practice daily living skills (like organizing your day, managing medications, budgeting, and cooking), social and communication skills, coping and stress management, and creating healthy routines.
We can also support your school or work goals, community participation, and connections with natural supports. PSR is person-centered and recovery-oriented, and it works alongside therapy, psychiatric care, and case management—so your plan feels connected, practical, and built around you.
Respite services offers short-term, planned or as-needed support that helps individuals remain safely in their own homes while giving family members and primary caregivers time to rest and recharge. Our trained VBCMH staff provide compassionate, reliable care—supporting daily routines, medication reminders, and meaningful activities—so your loved one is comfortable and engaged while you take care of yourself, other responsibilities, or appointments.
We tailor respite to fit your family’s needs, with options for in-home support or community-based settings, daytime or evening hours, weekdays or weekends. Your respite plan is person-centered and coordinated with broader VBCMH services, ensuring consistency and peace of mind. The goal is simple: reduce stress, strengthen stability at home, and help everyone in the household thrive.
Self-Determination means you have the right to make choices about your own life. The goal is full inclusion—feeling valued, connected, and part of your community—while reducing isolation for receiving mental health and wellness services.
Self-Determination and Self-Directed Services (SDS) is a way to plan and manage your supports yourself, instead of having everything set by professionals. You choose the services you want, who provides them, and when they happen. With help if you want it, you also decide how to use your approved service budget.
SDS is built on five simple ideas:
• Freedom: Choose what a good life looks like and the supports that fit you.
• Authority: Have a say in how your service dollars are used.
• Support: Organize people and resources in ways that are meaningful to you.
• Responsibility: Use funds wisely and hire trusted people or agencies; stay connected to your community.
• Confirmation: Be part of the decisions that affect your life—and help shape how services work.

Specialized Residential services provide safe, home-like settings with 24/7 support for individuals who need a higher level of care to live well in the community. In partnership with licensed providers, our team offers help with daily living skills, medication support, personal care, health appointments, and meaningful activities—always guided by a person-centered plan. We work closely with families, case managers, therapists, and psychiatric providers to create consistent routines, behavioral supports, and goals that build independence. The focus is dignity, stability, and progress—so each person has the right supports in place to feel at home and thrive.
Wraparound Services at VBCMH bring your family, your chosen supports, and community partners together to build one plan that fits real life. Personally guided and family-driven, Wraparound focuses on strengths and culture while coordinating help across home, work, and other systems. Your VBCMH Case Manager helps set goals, organize services, and create a practical crisis plan—with regular check-ins to keep things on track.
Meetings can happen where it works for you—at home, at work, at higher education institutions, or in the community—and we lean on natural supports to build skills and stability. The goal is simple: help adults of all ages stay safely at home and thrive in their community.
